INTRODUCTION AND OBJECTIVE: Among the many treatments for erectile dysfunction, implantation of a penile prosthesis has been associated with high patient satisfaction rates. However, patients with coexistent Peyronie's disease (PD) and refractory erectile dysfunction and/or severe deformities may show different results. The aim of our study was to assess and to compare the level of satisfaction, with an inflatable penile prosthesis (IPP), in men with/without coexistent PD. MATERIAL AND METHODS: A survey study based on a five-item satisfaction questionnaire was submitted to all those live patients implanted in the period 1992-2022 at our center (n = 570) and their partners. Ninety-two percent of implants were inflatable devices. Surgeries were mainly performed by two surgeons. The main outcome measure used was the level of patient and partner satisfaction with sexual intercourse after IPP. RESULTS: Of the 570 eligible patients, 479 (84%) completed the survey (393 Non-PD: GROUP 1; 70 non-complex PD-Group 2; 16 complex PD). Eighty-six per cent of patients in Group 1 reported satisfactory sexual intercourse (very or moderately satisfied). Non-complex PD implanted patients (Group 2) reported a global 81% satisfactory sexual intercourse (very or moderately satisfied) (p > 0.05). However, when we evaluated the PD subgroup of patients with severe PD who require incision/excision/grafting at the time of implant (Group 3: n = 20), only 61% reported satisfactory sexual intercourse (p < 0.01) with predominance of moderately satisfied patients over very satisfied: 78% vs. 22%). Additionally, 84% (Group 1), 80% (Group 2) and 54% (Group 3) of partners reported satisfactory intercourses, respectively (p < 0.01). Overall, 84% of Group 1 implants and 79% of Group 2 reported that they would undergo the procedure again if the IPP failed (p > 0.05; ns). Only 50% of Group 3 patients would do it again. With regard to cosmetic aspects, 48% of the Group 3 implant reported penile shortness or soft glans as the main causes of their dissatisfaction. Only 2.4% of total PP patients expressed difficulty in manipulating the device. CONCLUSION: The presence of PD alone may not impact PP patient and partner satisfaction, but patients with more severe baseline deformity who require incision/grafting may be less satisfied with outcomes including penile length and glans sensation.

Resumen El linfogranuloma venéreo es una infección de trans misión sexual (ITS) causada por las serovariedades L1, L2 y L3 de Chlamydia trachomatis. Una forma rara de presen tación del estadio primario es la linfangitis localizada en pene, con la formación de uno o más nódulos linfáticos tensos clásicamente llamados "bubonódulos" (bubón pe queño). Presentamos el caso de un paciente HIV positivo con conductas de riesgo para ITS con nódulos peneanos como manifestación de linfogranuloma venéreo.


Abstract Lymphogranuloma venereum is a sexually trans mitted disease caused by L1, L2 and L3 serovars of Chlamydia trachomatis. A rare clinical presentation of the primary stage is lymphangitis of the penis, with the appearance of one or more tense lymph nodes classically called "buboes". We report the case of an HIV-positive patient with sexually transmitted disease risk behaviors with penile nodules as a manifestation of lymphogranuloma venereum.

Objetivo: proporcionar reflexões sobre o cuidado de enfermagem com abordagem à sexualidade para a saúde do homem penectomizado por câncer de pênis. Conteúdo: corresponde a estudo reflexivo construído a partir de leituras em produções científicas sobre o cuidado de Enfermagem e a sexualidade para o paciente oncológico penectomizado, à luz dos pensamentos e pressupostos ontológicos e teóricos de Martin Heidegger. Considerações finais: as novas condições impostas pelo adoecimento por câncer de pênis e o tratamento mutilador, a penectomia, exigem uma Enfermagem que aborde a sexualidade, considerando o indivíduo em sua totalidade e com ênfase no cuidado com perspectiva na profundidade e subjetividade existenciais humanas.


Objective: to provide reflections on nursing care with an approach to sexuality for the health of men undergoing penectomy for penile cancer. Content: reflective study developed from readings in scientific productions on Nursing care and sexuality for the penectomized cancer patient, in the light of Martin Heidegger's ontological and theoretical thoughts and assumptions. Final considerations: the new conditions imposed by illness from penile cancer and the mutilating treatment, penectomy, require Nursing that addresses sexuality, considering the individual in its entirety and with an emphasis on care with a perspective on human existential depth and subjectivity.

Consideraciones finales: las nuevas condiciones impuestas por la enfermedad por cáncer de pene y el tratamiento mutilador, la penectomía, exigen una Enfermería que aborde la sexualidad, considerando al individuo en su totalidad y con énfasis en el cuidado con perspectiva en la profundidad y subjetividad existenciales humanas.

Introducción y objetivo La implantación de prótesis deZ pene (PP) es una alternativa eficaz para la disfunción eréctil. Aunque inicialmente la cirugía de PP se realizaba en régimen hospitalario, existe una tendencia creciente a realizar el implante de PP en un modelo de cirugía mayor ambulatoria (CMA). El objetivo de este estudio es realizar una revisión sistemática de la literatura para identificar la evidencia disponible sobre la implantación de PP en el marco de la CMA en comparación con el procedimiento realizado en régimen hospitalario. Material y métodos Se realizó una búsqueda en las bases de datos electrónicas PubMed, EMBASE, Cochrane Library y MEDES y en los suplementos no indexados de los congresos científicos para identificar artículos relacionados con la implantación quirúrgica de PP en CMA hasta febrero de 2021. Los términos de búsqueda incluyeron prótesis de pene, disfunción eréctil, cirugía ambulatoria, atención ambulatoria y cirugía. Resultados Entre las 171 publicaciones obtenidas (51 en PubMed, 73 en EMBASE, 3 en Cochrane, 2 mediante MEDES y 42 mediante búsqueda manual), se seleccionaron finalmente 5 estudios. No hubo diferencias significativas entre la CMA y el régimen hospitalario en términos del tipo de dispositivo, el abordaje quirúrgico o la ubicación del reservorio. Las tasas de complicaciones observadas en ambos grupos fueron similares. La implantación de PP en régimen de CMA supuso un menor coste que la cirugía en régimen hospitalario y se asoció con tasas aceptables de satisfacción de los pacientes y un adecuado control del dolor. Conclusiones Los estudios demostraron que la implantación de PP en régimen de CMA puede lograr resultados similares en términos de seguridad y satisfacción a la implantación de PP en el régimen hospitalario, pudiendo también reducir los costes y mejorar la eficiencia. Esta investigación podría ayudar a los responsables de la toma de decisiones a extender la cirugía de PP al régimen ambulatorio (AU)


Introduction and objective Penile prosthesis (PP) implantation is an effective option for erectile dysfunction. Although initially PP surgery was carried out in an inpatient setting, there is a growing trend to implant PP in a major ambulatory surgery (MAS). This study aimed to perform a systematic review of the literature to identify available evidence of the implantation of PP under MAS setting and go carry out a comparison between MAS and inpatient procedures. Material and methods PubMed, EMBASE, Cochrane Library and MEDES electronic databases and non-indexed supplements for scientific congresses were searched to identify articles related to the surgical implantation of PP in MAS up to February 2021. Key search terms included penile prosthesis, erectile dysfunction, ambulatory surgery, ambulatory care, and surgery. Results Among 171 publications retrieved (51 PubMed, 73 EMBASE, 3 Cochrane, 2 using MEDES and 42 manual searching), 5 studies were finally selected. There were no significant differences between MAS or inpatient setting in terms of the type of device, surgical approach, or location of reservoir. Complication rates observed in both groups were similar. Implantation of PP in MAS was less expensive than inpatient surgery and was associated with acceptable patient satisfaction rates and adequate pain control. Conclusions Studies demonstrated that outpatient PP surgery can achieve similar outcomes in terms of safety and satisfaction to implantation of PP in the inpatient setting, while it could reduce costs and improve the efficiency. This research could provide support decision makers to extend PP surgery into the ambulatory setting (AU)

Las máculas melanóticas del pene son lesiones idiopáticas, benignas, poco frecuentes, en ocasiones multifocales, heterocrómicas e irregulares, que deben diferenciarse del melanoma de mucosas. Se presenta el caso de un paciente con lesiones pigmentadas en el pene, asintomáticas, pero que se habían incrementado en número. Con la ayuda de la dermatoscopia y la histopatología, se llegó al diagnóstico de máculas melanóticas del pene. Aunque son reducidas las opciones terapéuticas en el medio descrito, la trascendencia del diagnóstico de esta dermatosis radica en descartar la posibilidad de su malignidad.


Melanotic macules of the penis are idiopathic, benign, rare lesions, sometimes multifocal, heterochromic and irregular, which must be differentiated from mucosal melanoma. A patient with asymptomatic pigmented lesions on the penis, but which had increased in number is presented. With dermoscopy and histopathology test, the diagnosis of melanotic macules of the penis was reached. Although the therapeutic options in the described environment are limited, the significance of this dermatosis diagnosis lies in ruling out the possibility of its malignancy.

Introducción y objetivo: La cirugía de alargamiento y engrosamiento de pene supone un reto técnico para el cirujano, puesto que las técnicas clásicas han demostrado resultados insuficientes para los pacientes. Nuestro objetivo es presentar nuestra experiencia con una técnica reproducible que permite evitar el empleo de extensores y las complicaciones de la infiltración de grasa. Material y método: Analizamos de forma retrospectiva 750 casos de alargamiento y engrosamiento de pene primarios realizados con la técnica NEF (No Extenders No Fat) - No Extensores No Grasa), analizando la técnica quirúrgica, sus resultados y complicaciones. Esta técnica se basa en la creación de un colgajo de avance cutáneo junto con un colgajo de fascia de Scarpa y un nanofat infiltrado en las fascias profundas del pene. Resultados: La ganancia media en longitud en el grupo de pacientes estudiados fue de 4.8cm en reposo, con un espectro de 2 hasta 8.1cm. En el engrosamiento, se alcanzó un incremento medio de 1.17 cm (mínimo de 0.5 y máximo de 1.8 cm). Conclusiones: La tasa de satisfacción de pacientes, de complicaciones y retoques en nuestro grupo de estudio nos hablan de una cirugía que mejora las técnicas previas, más invasivas y con mayor tasa de complicaciones. El apoyo psicológico y la ayuda profesional preoperatoria es imperativa, y se debe ofrecer especialmente en casos de dismorfofobia. El trabajo pre y postoperatorio es esencial en estos pacientes. Conseguir pacientes con expectativas reales es la primera barrera que debemos franquear. Solo cuando el paciente haya entendido la cirugía y sus resultados, debemos avanzar hacia ella. (AU)


Background and objective: Penis lengthening and thickening surgery is a technical challenge for the surgeon, since classical techniques have shown insufficient results for patients. Our objective is to present our experience in the develop of a a reproducible technique that avoids the use of extensors and the complications of fat infiltration. Methods: Retrospectiv análisis of 750 cases of primary penis enlargement and thickening performed with the NEF (No Extenders No Fat) technique, analyzing the surgical technique, its results and complications. This technique is based on the creation of a skin advancement flap, together with a Scarpa fascia flap and a nanofat infiltrated at the deep fascia of the penis. Results: Mean gain in length in the group of patients studied was 4.8 cm at rest with a spectrum from 2 to 8.1 cm. In thickening, the measurement reached an average increase of 1.17 cm (minimum of 0.5 and a maximum of 1.8 cm). Conclusions: Rates of patient satisfaction, complications and retouching in our group tell us about a surgery that improves previous more invasive techniques with a higher rate of complications. Psychological support and professional pre-surgery help is imperative and should be offered especially in those cases of dysmorphophobia. Pre and postoperative work is essential in these patients. Getting patients with real expectations is the first barrier we must overcome. Only when the patient has understood the surgery and its results, should we move towards it. (AU)

OBJECTIVE: Congenital penile curvature is defined as the non-straightness of the penis without any urethral or penile pathology. We aimed to evaluate the factors that cause penile shortening after plication surgery in patients with congenital penile curvature. METHODS: Between November 2010 and December 2020, we retrospectively reviewed patients with CPC undergoing tunica albuginea plication surgery. Before the procedure, patients' age, curvature location and degrees, as well as penile length were recorded. After the treatment, penile lengths were measured and recorded again. Early and late period results were recorded. RESULTS: Plication surgery was performed in 130 patients. The median age was 24 years. Seventy-six patients had ventral curvature, 22 had dorsal curvature, 32 had lateral curvature. Average shortening of penile length in patients with curvature below 30° was: ventral 8-16 mm, dorsal 6-13 mm, lateral 5-12 mm. Patients with curvatures above 30° were: ventral 12-22 mm, dorsal 8-20 mm, lateral 2-12 mm. CONCLUSION: Penile length shortening after plication is inevitable. Curvature degree and direction are factors affecting penile length after surgery. Therefore, patients and relatives should be informed in more detail about this complication.

Introduction and objectives: Comparison of early period sexual function parameters in patients who had surgical repair and conservative follow-up after penile fracture and the evaluation of surgical intervention time on these parameters were planned in this study. Materials and methods: Total of 26 patients who were treated for penile fracture were evaluated. 19 patients had surgical repair and 7 patients had conservative treatment.Sexual function and erectile dysfunction (ED) degree of the patients before penile fracture and in the 12th week after fracture were evaluated with 5-question International Index of Erectile Function (IIEF-5) questionnaire, Erection Hardness Score (EHS), Sexual Encounter Profile(SEP) 2 and SEP 3. Parameters showing sexual function before and after the fracture were compared. Results: In both groups, a significant change was detected in IIEF-5 score, EHS, SEP-2 and SEP-3 parameters of the patients measured after penile fracture compared to the values before the fracture (all parameters, p<0.05). No difference was detected in the parameters measured before and after the fracture among surgical repair and conservative treatment groups (all parameters p>0.05). Mean time passing until the surgery after fracture was measured as 9.6±6.85h in 19 patients who had surgery. Conclusion: A difference wasn’t detected in sexual parameters in conservative treatment and surgical repair groups in this study. As a significant decrease was observed in sexual function parameters even in conservative treatment cases without sudden detumescence and tunica albuginea rupturing, we think that quick surgical exploration would be useful in cases considered to have penile fracture. Introducción y objetivos: En este estudio se planificó la comparación de los parámetros de función sexual en el período temprano en pacientes sometidos a reparación quirúrgica y seguimiento conservador después de una fractura de pene y la evaluación del tiempo de intervención quirúrgica sobre estos parámetros. Materiales y métodos: Se evaluaron un total de 26 pacientes que fueron tratados por fractura de pene: 19 fueron sometidos a reparación quirúrgica y 7 tuvieron un seguimiento conservador. La función sexual y el grado de disfunción eréctil de los pacientes antes de la fractura de pene y en la semana 12 después de la fractura se evaluaron con el cuestionario International Index of Erectile Function de 5 preguntas (IIEF-5), el Erection Hardness Score (EHS), y el Sexual Encounter Profile (SEP) 2 y SEP 3. Se compararon los parámetros que muestran la función sexual antes y después de la fractura. Resultados: En ambos grupos se detectó un cambio significativo en la puntuación IIEF-5 y los parámetros EHS, SEP 2 y SEP 3 de los pacientes medidos después de la fractura de pene en comparación con los valores antes de la fractura (todos los parámetros, p<0,05). No se detectaron diferencias en los parámetros medidos antes y después de la fractura entre los grupos de reparación quirúrgica y tratamiento conservador (todos los parámetros p>0,05). El tiempo medio transcurrido hasta la cirugía después de la fractura se midió como 9,6±6,85h en los 19 pacientes intervenidos. Conclusión: No se detectaron diferencias en los parámetros sexuales entre los grupos de observancia conservadora y reparación quirúrgica en este estudio. Como se observó una disminución significativa de los parámetros de función sexual incluso en casos de seguimiento conservador sin detumescencia súbita y desgarro de la túnica albugínea, pensamos que la exploración quirúrgica rápida sería útil en los casos en los que se considere que tienen fractura de pene. Objective: To describe the clinical behavior of human papillomavirus in men. Materials and methods: Current international literature was reviewed to describe the clinical behavior of human papillomavirus in men. Results: Internationally, the overall prevalence of HPV DNA is 50.8%, HPV considered high risk are 14 types. Prevalence of HPV DNA in invasive penile cancer ranges from 33.1% to 47%. HPV-16 has been the most frequent (68.3%), followed by HPV-6 (8.1%) and HPV-18 (6.9%). Positive HPV is described as an independent prognostic factor for cancer-specific survival. Conclusion: It is not clear why HPV infection has a predilection in specific areas of the genital tract. However, it is important to note that there are factors that increase the risk of HPV infection. (AU)


Objetivo: Describir el comportamiento clínico del virus del papiloma humano en hombres. Materiales y métodos: Se revisó la literatura internacional actual para describir el comportamiento clínico del virus del papiloma humano en los hombres. Resultados: En el ámbito internacional, la prevalencia general del ADN del VPH es del 50,8%. Los VPH considerados de alto riesgo son 14 tipos. La prevalencia del ADN del VPH en el cáncer de pene invasivo oscila entre el 33,1% y el 47%, siendo el VPH-16 el más frecuente (68,3%), seguido del VPH-6 (8,1%) y del VPH-18 (6,9%). El VPH positivo se describe como un factor pronóstico independiente para la supervivencia específica del cáncer. Conclusión: No está claro por qué la infección por VPH muestra predilección por áreas específicas del tracto genital. Sin embargo, es importante tener en cuenta que existen factores que aumentan el riesgo de infección por VPH. (AU)

Objetivo La incurvación peneana congénita se define como la incurvación del pene en ausencia de patología uretral o peneana. Nuestro objetivo es evaluar los factores causantes del acortamiento del pene después de la plicatura en pacientes con incurvación peneana congénita. Métodos Entre noviembre de 2010 y diciembre de 2020 revisamos de forma retrospectiva a los pacientes con incurvación peneana congénita que se sometieron a una cirugía mediante técnica de plicatura de la túnica albugínea. Antes del procedimiento, se registró la edad de los pacientes, la ubicación y los grados de la desviación y la longitud del pene. Tras el tratamiento se midió y registró de nuevo la longitud del pene. Se registraron los resultados de los períodos inicial y final. Resultado Se realizó cirugía con plicatura en 130 pacientes. La media de edad era de 24 años. Setenta y seis pacientes tenían incurvación ventral, 22 dorsal y 32 lateral. El acortamiento medio de la longitud del pene en aquellos pacientes con incurvación inferior a 30° fue: ventral 8-16 mm, dorsal 6-13 mm, lateral 5-12 mm. En los pacientes con incurvación superior a 30° fue: ventral 12-22 mm, dorsal 8-20 mm, lateral 2-12 mm. Conclusión El acortamiento del pene después de la plicatura es inevitable. El grado y la dirección de la desviación son factores que afectan la longitud del pene después de la operación. Por lo tanto, los pacientes y sus familiares deben recibir información detallada sobre esta complicación (AU)

SUMMARY: The corporo-glans ligament is the ligament connecting the corpus cavernosum and the glans of the penis. The anatomical description of the corporo-glans ligaments shape is still uncertain, this knowledge affects penile reconstructive procedures. The anatomy of the corporo-glans ligament was analyzed and recorded via observing sagittal sections of 10 different penile P45 plastination sections. According to the P45 plastination sections, the corporo-glans junction displayed a fibrous tissue band connecting the distal ends of the two corpus cavernous (CC) with the glans penis (GP). The fibrous band was a round-obtuse shape and ran deep into the glans of the penis and occupied about 2/3 of the whole GP. The original end was laid in a socket embedded in the GP. The density of the fibers of the ligament at the original end close to the tunica albuginea was less than that of the other parts. The fibers originating from the tunica albuginea, directly extended to the blind end of the two CC, covering the distal end of the two CC.

La anatomía del ligamento cuerpo cavernoso-glande se analizó y registró mediante la observación de 10 secciones sagitales diferentes del pene a través de plastinación P45. Según las secciones de plastinación, la unión cuerpo-glande mostraba una banda de tejido fibroso que conectaba los extremos distales de los dos cuerpos cavernosos con el glande del pene. La banda fibrosa tenía una forma redonda y obtusa y se adentraba profundamente en el glande del pene ocupando alrededor de 2/3 de él. En su origen se coloca en un espacio profundo en el glande del pene. La densidad de las fibras del ligamento cuerpo cavernoso-glande en su origen cercano a la túnica albugínea era menor que el de las otras partes. Las fibras que se originan en la túnica albugínea, se extienden directamente hasta el extremo ciego de los dos cuerpos cavernosos, cubriendo el extremo distal de estos.

Presentación del caso. Paciente masculino de 52 años que se presentó a la consulta de urología con historia de dos años de notar una lesión en el glande y el prepucio, de color rojo brillante, pruriginosa y dolorosa con aumento progresivo del tamaño que no mejoró con tratamientos antibióticos y anti fúngicos. Intervención terapéutica. Se realizó una glandectomía parcial con injerto de piel de muslo. Evolución clínica. Luego de un mes, el injerto presentó un 95 % de acoplamiento. No se observó recurrencia local de cáncer. El estudio histopatológico reportó un carcinoma escamoso invasor en la lesión del prepucio y en la piel del glande, con todos los márgenes quirúrgicos, limites laterales y profundos, negativos a malignidad. Luego de ocho meses posquirúrgicos, se observó el recubrimiento del glande con un adecuado resultado estético, con apariencia similar a la cubierta natural


Case presentation. A A 52-year-old male patient presented to the urology office with a two-year history of noticing a bright red, pruritic, and painful lesion on the glans and foreskin with a progressive increase in size that did not improve with antibiotic and antifungal treatments. Treatment. Partial glandectomy with thigh skin graft was performed. Outcome. After After one month, the graft presented a 95 % of coupling. No local recurrence of cancer was observed. The histopathological study reported invasive squamous cell carcinoma in the lesion of the foreskin and glans skin, with all surgical margins, lateral and deep limits, negative for malignancy. After eight months post-surgery, the covering of the glans was observed with an appropriate esthetic result, with a similar appearance to the natural covering

OBJECTIVE: To describe the clinical behavior of human papillomavirus in men. MATERIALS AND METHODS: Current international literature was reviewed to describe the clinical behavior of human papillomavirus in men. RESULTS: Internationally, the overall prevalence of HPV DNA is 50.8%, HPV considered high risk are 14 types. Prevalence of HPV DNA in invasive penile cancer ranges from 33.1% to 47%. HPV-16 has been the most frequent (68.3%), followed by HPV-6 (8.1%) and HPV-18 (6.9%). Positive HPV is described as an independent prognostic factor for cancer-specific survival. CONCLUSION: It is not clear why HPV infection has a predilection in specific areas of the genital tract. However, it is important to note that there are factors that increase the risk of HPV infection.

INTRODUCTION AND OBJECTIVE: Penile prosthesis (PP) implantation is an effective option for erectile dysfunction. Although initially PP surgery was carried out in an inpatient setting, there is a growing trend to implant PP as a major ambulatory surgery (MAS). This study aimed to perform a systematic review of the literature to identify available evidence of the implantation of PP under MAS setting and go carry out a comparison between MAS and inpatient procedures. MATERIAL AND METHODS: PubMed, EMBASE, Cochrane Library and MEDES electronic databases and non-indexed supplements for scientific congresses were searched to identify articles related to the surgical implantation of PP in MAS up to February 2021. Key search terms included penile prosthesis, erectile dysfunction, ambulatory surgery, ambulatory care, and surgery. RESULTS: Among 171 publications retrieved (51 PubMed, 73 EMBASE, 3 Cochrane, 2 using MEDES and 42 manual searching), 5 studies were finally selected. There were no significant differences between MAS or inpatient setting in terms of the type of device, surgical approach, or location of reservoir. Complication rates observed in both groups were similar. Implantation of PP in MAS was less expensive than inpatient surgery and was associated with acceptable patient satisfaction rates and adequate pain control. CONCLUSIONS: Studies demonstrated that outpatient PP surgery can achieve similar outcomes in terms of safety and satisfaction to implantation of PP in the inpatient setting, while it could reduce costs and improve the efficiency. This research could support decision makers to extend PP surgery into the ambulatory setting.

INTRODUCTION AND OBJECTIVES: Comparison of early period sexual function parameters in patients who had surgical repair and conservative follow-up after penile fracture and the evaluation of surgical intervention time on these parameters were planned in this study. MATERIALS AND METHODS: Total of 26 patients who were treated for penile fracture were evaluated. 19 patients had surgical repair and 7 patients had conservative treatment. Sexual function and erectile dysfunction (ED) degree of the patients before penile fracture and in the 12th week after fracture were evaluated with 5-question International Index of Erectile Function (IIEF-5) questionnaire, Erection Hardness Score (EHS), Sexual Encounter Profile(SEP) 2 and SEP 3. Parameters showing sexual function before and after the fracture were compared. RESULTS: In both groups, a significant change was detected in IIEF-5 score, EHS, SEP-2 and SEP-3 parameters of the patients measured after penile fracture compared to the values before the fracture (all parameters, p<0.05). No difference was detected in the parameters measured before and after the fracture among surgical repair and conservative treatment groups (all parameters p>0.05). Mean time passing until the surgery after fracture was measured as 9.6±6.85h in 19 patients who had surgery. CONCLUSION: A difference wasn't detected in sexual parameters in conservative treatment and surgical repair groups in this study. As a significant decrease was observed in sexual function parameters even in conservative treatment cases without sudden detumescence and tunica albuginea rupturing, we think that quick surgical exploration would be useful in cases considered to have penile fracture.

Objetivo: La disfunción venooclusiva peneana se reconoce como causa de disfunción eréctil en algunos pacientes jóvenes sin otros factores de riesgo. El objetivo del estudio es reportar los resultados de una cirugía de esta patología en pacientes menores de 40 años, con seguimiento hasta 6 meses postratamiento. Método: Estudio descriptivo en una cohorte retrospectiva de historias clínicas con 50 pacientes que cumplían criterios de inclusión, evaluados y sometidos a cirugía estandarizada por el mismo cirujano entre 01/2014 y 10/2021. Al grupo se le practicó un puntaje de síntomas (Sexual Health Inventory for Men [SHIM]) antes de la cirugía y a los 3 y 6 meses de esta. Los resultados del SHIM pre- y posoperatorio se relacionaron con los diferentes grados de disfunción eréctil (grave, moderada y leve), y adicionalmente se dividieron en tres grupos según criterios definidos en exitoso, moderado y deficiente. El diagnóstico de fugas venosas se hizo mediante ecografía Doppler peneana con vasoactivo, registrando fugas venosas, grado de fibrosis peneana y presencia de glande blando. El protocolo del estudio fue aprobado por el Comité de Investigación del Hospital Pablo Tobón Uribe, de Medellín. Resultados: El rango de edad al momento del procedimiento fue de 18-50 años, con una edad media de 31,2 años, y el inicio de la disfunción eréctil a los 23,4 años. El tiempo de evolución de la enfermedad antes de intentar la cirugía varió entre 2 y 21 años (7,5 años en promedio). El estado de disfunción eréctil según el SHIM preoperatorio fue 4 (8%) grave, 46 (92%) moderada y no hubo ningún caso leve. Los desenlaces posoperatorios a 6 meses fueron 11 (22%) moderada, 20 (40%) leve y 19 (38%) sin disfunción eréctil. Adicionalmente, según análisis interno al SHIM, 34 (68%) fueron exitosos, 6 (12%) moderados y 10 (20%) deficientes. Conclusiones: La ligadura de fugas venosas dorsales peneanas tiene buenos resultados a mediano plazo en cuanto a calidad de la erección y satisfacción del paciente.


Objective: Penile venocclusive dysfunction is recognized as a cause of erectile dysfunction in some young patients with no other risk factors. The objective of the study is to report the results of surgery for this pathology in patients under 40 years of age, with follow-up up to 6 months post-treatment. Method: A descriptive study was carried out in a retrospective cohort of medical records with 50 patients with inclusion criteria, evaluated and submitted to standardized surgery by the same surgeon between 01/2014 and 10/2021. A symptom score (Sexual Health Inventory for Men [SHIM]) was applied to the group, before surgery, and 3 and 6 months after. The pre- and post-operative SHIM results have been related to the different degrees of erectile dysfunction and, additionally, they were divided into three groups according to criteria defined as successful, moderate, and poor. The diagnosis of venous leaks was made by vasoactive penile Doppler ultrasound, recording venous leaks, degree of penile fibrosis and presence of soft glans penis. The study protocol was approved by the Research Committee of the Pablo Tobon Uribe Hospital, in Medellín. Results: Age range at the time of the procedure 18-50 years, mean age 31.2 years and onset of erectile dysfunction 23.4 years. Time of evolution of the disease before attempting surgery varied between 2 and 21 years (average 7.5 years). Erection status according to the preoperative SHIM was severe erectile dysfunction 4 (8%) and moderate 46 (92%). The outcomes at 6 months were 11 (22%) with moderate erectile dysfunction, 20 (40%) switched to mild and19 (38%) were left without erectile dysfunction. Additionally, according to additional analysis to the SHIM, 34 (68%) were successful, 6 (12%) moderate and 10 (20%) poor. Conclusions: Ligation of penile dorsal venous leaks has good medium-term results in terms of erection quality and patient satisfaction.

Os Papilomavírus Humano (HPVs) são membros da família Papilomaviridae. O vírus destaca-se pelo seu tropismo por células epiteliais, infectando exclusivamente mucosa epitelial e cutânea. O HPV-16 e HPV-18 são subtipos classificados como de alto risco, conhecidos por sua oncogenicidade, fortemente associados aos cânceres anais, genitais e de orofaringe. Lesões por HPV representam um grande grupo de doenças sexualmente transmissíveis. O objetivo do presente estudo consistiu em realizar uma revisão narrativa sobre a associação entre lesões por HPV e carcinomas genitais e da cavidade oral. Realizamos uma busca na base de dados eletrônicos PubMed, Lilacs, Scielo, Medline e Google Scholar, sendo utilizados artigos publicados entre os anos de 2017-2021, ao fim, foram selecionados 36 artigos. Grande parte das infecções por HPV são subclínicas, ou seja, não apresentam sintomatologia importante e tendem a desaparecer espontaneamente. Desta forma, faz-se necessário ter conhecimento a respeito dos aspectos clínicos e comportamentais dessas lesões, possibilitando o diagnóstico precoce, evitando a evolução para estágios mais invasivos, favorecendo um tratamento efetivo e melhor prognóstico.


Human Papillomaviruses (HPVs) are members of the Papilomaviridae family. The virus stands out for its tropism for epithelial cells, exclusively infecting epithelial and cutaneous mucosa. O HPV-16 and HPV-18 are subtypes classified as high risk, known for their oncogenicity, strongly associated with anal, genital and oropharyngeal cancers. HPV lesions represent a large group of sexually transmitted diseases. The objective of this study was to carry out a narrative review on the association between HPV lesions and genital and oral cavity carcinomas. We carried out a search in the electronic databases PubMed, Lilacs, Scielo, Medline and Google Scholar, using articles published between the years of 2017-2021, at the end, foram selected 36 articles. A large part of HPV infections are subclinical, or seem to, do not present significant symptoms and tend to disappear spontaneously. In this way, it is necessary to be aware of the two clinical and behavioral aspects of these injuries, enabling early diagnosis, avoiding evolution to more invasive stages, favoring effective treatment and better prognosis.


Los virus del papiloma humano (VPH) son miembros de la familia Papillomaviridae. El virus destaca por su tropismo por las células epiteliales, infectando exclusivamente mucosas epiteliales y cutáneas. El VPH-16 y el VPH-18 son subtipos clasificados como de alto riesgo, conocidos por su oncogenicidad, fuertemente asociados con cánceres anales, genitales y orofaríngeos. Las lesiones por VPH representan un gran grupo de enfermedades de transmisión sexual. El objetivo del presente estudio fue realizar una revisión narrativa sobre la asociación entre las lesiones por VPH y los carcinomas genitales y de cavidad oral. Introducción: El cáncer de pene es una neoplasia maligna poco frecuente en todo el mundo; representa el 1 % de todos los cánceres que afectan a los hombres. Es una neoplasia caracterizada por un proceso proliferativo de células epiteliales, originándose a partir de la piel del prepucio interno o del glande, que se identifica por un crecimiento invasivo y diseminación metastásica temprana a ganglios linfáticos. Objetivo: Exponer la presentación clínica, diagnóstico y tratamiento de un hombre joven con cáncer de pene. Caso clínico: Paciente masculino de 31 años de edad, color de la piel blanca, con antecedentes de salud anterior y de procedencia urbana; que desde hace dos años presenta lesión ulcerosa, descamativa, granulosa en el pene; no dolorosa, con secreciones fétidas y que ha ido aumentando de tamaño. Con la aplicación de anestesia regional espinal se realizó penectomía parcial y se confirmó el diagnóstico de un carcinoma de células escamosas de pene. Conclusiones: El carcinoma de células escamosas de pene es infrecuente en hombres jóvenes y de buen pronóstico si es diagnosticado a tiempo y para cuyo diagnóstico se precisa ser especialmente estricto con los criterios histológicos. La penectomía parcial es el tratamiento de elección de la lesión primaria. El seguimiento de estos pacientes es fundamental para actuar de forma rápida ante la presencia de recidiva o adenopatías metastásicas.


Introduction : Cancer of the penis is a rare malignant neoplasm worldwide; it represents 1 % of all cancers that affect men. It is a neoplasm characterized by a proliferative process of epithelial cells, originating from the skin of the inner prepuce or glans penis, which is identified by invasive growth and early metastatic spread to lymph nodes. Objective : To expose the clinical presentation, diagnosis and treatment of a young man with penile cancer. Clinical case : Male patient 31 years of age, white skin color, with a history of previous health and urban origin; that for two years has had ulcerative, decamative, granulous lesion on the penis; not painful, with fetid secretions and that has been increasing in size. With the application of regional spinal anesthesia, partial penectomy was performed and the diagnosis of squamous cell carcinoma of the penis was confirmed. Conclusions : Squamous cell carcinoma of the penis is uncommon in young men and has a good prognosis if it is diagnosed early and for whose diagnosis it is necessary to be especially strict with the histological criteria. Partial penectomy is the treatment of choice for the primary lesion. The follow-up of these patients is essential to act quickly in the presence of recurrence or metastatic adenopathies.

OBJECTIVES: In the present study, we aimed to investigate the effect of dexpanthenol on wound healing at the histopathological level on cavernous tissue. MATERIALS AND METHODS: Forty-four Wistar albino rats weighing 220-250 g were used. The rats were randomly divided into four groups as Group B, Group S, Group LD, and Group SD. In Group B, the incision was not repaired and left to secondary healing. In Group S, the incision line was repaired with 5/0 polyglactin suture. In Group LD, 0.25 mg/kg dexpanthenol was applied subcutaneously below the repaired wound region once a day during 14 days. In Group SD, 500 mg dexpanthenol was applied intraperitoneally once a day during 14 days. RESULTS: No fibrosis was observed in 8 (80%) rats in group SD. Fibrosis rates were significantly lower in Group SD compared to Group B, Group S, and Group LD (p = 0.013, p = 0.005, and p = 0.003, respectively). CONCLUSION: Systemic dexpanthenol administration significantly decreased fibrosis in penile fracture model on rats.


OBJETIVO: En el estudio actual nuestro objetivo fue investigar el efecto del dexpantenol en la cicatrización de heridas a nivel histopatológico en el tejido cavernoso. MÉTODOS: se utilizaron 44 ratas Wistar albinas con un peso de 220-250 g. Las ratas se dividieron aleatoriamente en 4 grupos como grupo B, grupo S, grupo LD y grupo SD. En el grupo B, la incisión no se reparó y se dejó para la cicatrización secundaria. En el grupo S, la línea de incisión se reparó con sutura de poliglactina 5/0. En el grupo LD, se aplicaron 0.25 mg/kg de dexpentanol por vía subcutánea debajo de la región de la herida reparada una vez al día durante 14 días. En el grupo SD se aplicaron 500 mg de dexpentanol por vía intraperitoneal una vez al día durante 14 días. RESULTADOS: No se observó fibrosis en 8 (80%) ratas del grupo SD. Las tasas de fibrosis fueron significativamente más bajas en el grupo SD en comparación con el grupo B, el grupo S y el grupo LD (todos p < 0.05). CONCLUSIÓN: La administración sistémica de dexpantenol disminuyó significativamente la fibrosis en el modelo de fractura de pene en ratas.

Introduction and Objective: The penile metastasis is a rare clinical entity. The objective is to present the first documented case report of penile metastasis from right colon. Clinical Case: A case of a 78-year-old man who was diagnosed with penile metastasis from right colon. The patient came to our consultation complaining of colic pain in the kidney and swelling of the penile which finally result in a malignant priapism. The diagnosis was histopathologic and was treated with chemotherapy and died few months later. Conclusion: Metastatic lesions in the penile are extremely rare; only 300 cases have been reported in the literature. It is a sign of bad prognosis. The mechanism of metastatic spread to the penis is not well established. Even there are several treatment options, is usually paliative (AU)


Introducción y Objetivo: La metástasis penena esuna entidad muy infrecuente. El objetivo es la presentacióndel primer caso documentado en la literatura de metástasispenana con origen en el colon derecho.Caso Clínico: Presentamos a un varón diagnosticadode priapismo producido por una metástasis peneana cuyotumor primitivo tiene lugar en el colon derecho. El pacienteacudió por dolor en fosa renal y éstasis venoso en el peneque finalmente le provocó un priapismo por afectación tumoral. El diagnóstico fue anatomopatológico, siendo el paciente tratado con quimioterapia.Conclusión: La presencia de metástasis en el penees una presentación clínica infrecuente y un signo de malpronóstico. El mecanismo fisiopatológico no está determinado. El tratamiento, aunque variado, es generalmente paliativo (AU)
